If they see no problems, and you meet the pre/post surgery assessments, it's submitted "qualified, information only". If they do see a problem, they elevate it. Read, read, read, read. I did all this back in 2004-2006 when it was still very fluid. I knew more about the process than the flight surgeon, because it was changing rapidly, and he wasn't up to speed on it yet. (i.e. he was telling me it was an exception to policy, while it was a local waiver at the time...) These questions should be coming after reading everything you can find.
