What your survey doesn't address is the human issue. Pilot error, in one way or another, is responsible for the vast majority of accidents. Single engine/multi; IFR/VFR; 1 pilot or 2 pilot crew, and to a much lesser extent, days versus nights, with night operations resulting in 3 to 4 times the day time accident rate. Equipment is not the answer. For every equipment benefit, there is an increased training and management cost that needs to be considered.
