Jester2138 Posted June 18, 2014 Posted June 18, 2014 I'm getting PRK done in about a month and want to make sure I don't mess anything up for a WOFT packet a year or two from now.Do I need to see a recruiter or are any forms, etc. that need to be completed something I can get online?I found a link to a few forms in Lindsey's awesome Guide (http://www.tamc.amedd.army.mil/offices/Ophthalmology/docs/2010/Army%20Aviation%20Refractive%20Surgery%20Policy.pdf). They looked pretty old, though. Is this all I need? Looks like something the doc fills out post-op. Quote
Lindsey Posted June 18, 2014 Posted June 18, 2014 Just make sure you meet the required pre-op refraction requirements which are in AR 40-501. Those forms in my guide are from 2007 and from a quick check it appears they are still good to go. You may be able to call Mr. Garcia and inquire about it, but he may not be able to affirm in that much detail. He's more there to let you know if your flight phys is good after completion. After you get the procedure, ensure you follow the checklist on those forms for all the post-op tests that are required. One or a couple may not be automatically done by your surgeon, so ensure he does it and at the required time hacks post-op. 1 Quote
BM1 Posted June 19, 2014 Posted June 19, 2014 Go in with the forms required for the flight physical from the beginning, it lets them know what your expecting on their end. My place was surprised by how detailed it was (like the serial number of the machine used) but none of it was a problem. They even included extra out patient visits for me for free just to make sure we got all the post op checkups in that the Army is looking for. I'm specifically referring to Appendix #3 Checklist for Eye Care provider. Quote
